Registration Forms

In order to register:

• You must be a registered member of St. Philip’s Parish.
• If you have been in our program, all work must be completed from the previous year in order to go to the next grade.

Please fill out one form for each child. Extra registration forms are in the main lobby of the Church.

Baptismal Certificates:
Certificates are needed for students in First Communion and Confirmation Classes. Please attach baptismal certificate to the registration form.
